Post Acute Partners, owner of Elderwood since July 2013, was established in January 2010 in response to national health care reform and the Accountable Care Act. Founders Dr. Jeffrey Rubin and Warren Cole believe the most effective way to provide post acute care is to integrate those services through common ownership, offering a full continuum of care. Post Acute Partners acquires geographically concentrated companies, which are already highly regarded for quality, then invests heavily in those organizations in the areas of clinical excellence, customer satisfaction, staff training and technology.
Established in 1978, Elderwood has grown to become the largest network of rehabilitation, skilled nursing, assisted living and independent living communities in Western New York, with 17 facilities, 2,800 beds and 4,000 employees. Elderwood is well known to, and highly regarded by, the hospitals and physician groups in the communities that each location serves. Customer satisfaction is rigorously tracked through robust surveys and interviews. Based on our latest data, 96 percent of residents said they were pleased with the care they received at an Elderwood community. Elderwood’s continuum of care also includes advanced specialized services in oncology, cardiac rehabilitation, ventilator care, IV therapy and wound care.
